LIABILITY WAIVER & RELEASE FOR HSP CONSULTATION FOR HSP COPING SKILLS
Please read carefully
I understand and certify that I am learning from Ted Zeff, Ph.D. only techniques for coping as an HSP (highly sensitive person). I understand that Dr. Ted Zeff is not a health provider and that the phone consultation is not to obtain information for my personal physical or emotional health. This HSP consultation is to obtain educational information and not for providing personal health advice.
I understand and certify that Dr. Zeff is not treating any physical or emotional condition that I may have. I am totally responsible if I choose to implement any techniques that I learn from Dr. Zeff and realize I need not accept any educational information that Dr. Zeff presents about HSPs. I will not discontinue any emotional or physical treatment that I am currently receiving as a result of this HSP consultation. I agree to consult my physician before implementing any techniques that Dr. Zeff discusses in his HSP consultation.
If the information presented in the HSP consultation is used without consultation and approval of a physician, the individual will be diagnosing for him or herself. No responsibility is assumed by Dr. Zeff for the use of the information contained in the class in lieu of a doctor's services. Some of the herbs and supplements discussed in the HSP consultation may be dangerous when improperly used. Do not attempt to self-treat based on information in the HSP consultation. If the information presented is used without conferring with and approval of a physician, the individual will be diagnosing for him/herself. The information presented in the HSP consultation by Dr. Zeff is not intended to diagnose, treat, cure or prevent any disease.
I have carefully read this agreement and fully understand its content before I begin Dr. Zeff's HSP consultation. I am aware that this is a waiver and release of potential liability of Dr. Zeff.
